Templates for registered nurse portfolio

***Please note we have updated Performance review documents for all nurses for all levels***

If you are not familiar with these please discuss with your manager
For nurses from external organisations only complete Part B for your PDRP.

*Please also note the change to the peer assessment requirement*

Peer assessments no longer need to be written examples of the nurses’ practice. Instead they will require an attestation (formal verification/agreement) that this is a true reflection of the nurse’s practice (please find the form in the list of documents below). This is together with the role of the peer assessor, which includes reading through the nurse’s examples and giving feedback if required.
