Templates for enrolled nurse portfolio

For nurses from external organisations only complete Part B for your PDRP

 *Please also note the change to the peer assessment requirement*

Peer assessments no longer need to be written examples of the nurses’ practice. Instead they will require an attestation (formal verification/agreement) that this is a true reflection of the nurse’s practice (please find the form in the list of documents below). This is together with the role of the peer assessor, which includes reading through the nurse’s examples and giving feedback if required.

4.  CC Annual Performance Review with Full Self and Peer Assessment EN Competent

5.  Evidence of practice hours

6.  Professional development career plan (PDCP)

7.  Professional development record (PDR)

8.  Curriculum Vitae (optional)

9.  Declaration/ verification (by a senior nurse/ employer)

Proficient portfolio requirements

Portfolio submissions

Capital and Coast completed portfolios for application to Accomplished level must be received by the Nurse Coordinator - Professional Development by the 1st of the month to be assessed at that month's PDRP panel. Accomplished portfolios should be sent to:
