Kapiti PHO
Kapiti PHO is a not-for-profit charitable trust that aims to ease the barriers to accessing primary care services. The PHO serves over 33,000 people on the Kapiti Coast from Paekakariki to Peka Peka Road just north of Waikanae. The PHO brings together a number of partner groups, including:
- The Kapiti Community Health Trust
- Te Runanga o Ati Awa ki Whakarongotai
- Hora Te Pai Health Services
- Ten general practices from across the Kapiti coast
- WIPA (KPHO's contracted Management Services Provider)
The role of Kapiti PHO’s board is to oversee the setting of policy, provide direction to management and monitor progress and performance. The Board meet monthly and meetings are open to the public. Member of the Board include:
- Chris Kerr, Chair, Hora Te Pai representative
- Dr Dermot O'Connor, general practice representative
- Hilda Broadhurst, Iwi representative
- Alison Chappell, community representative
- Pania Shingleton, community representative
- Dr Maeburn Staniland, general practice representative
- Helen Dryden, nursing representative
- Glenda Henshilwood, general practice representative
- Dr Brent Kriven, general practice representative.
- Bronagh Moloney, co-opted member, youth
- Bridget Taylor, co-opted member, child
The partnership model of the PHO has been extended to having memoranda of understanding with:
- local Iwi
- community
- Royal New Zealand Plunket Society
- Kapiti Coast District Council
Kapiti PHO has also implemented a number of services to respond to the needs of its community, including the following:
- Implemented a medication management programme
- Employed an outreach nurse to work with high need patients in the community
- Employed an Health Promoter to facilitate collaboration across the Kapiti community
- Provided free melanoma spot checks during Sunsmart Week
- Implemented Care Plus for high-need patients with complex health needs
- Door-to-door transport service.
