LTC Self Management REOI

C&C DHB Request Expressions of Interest in providing self management support approaches for long term conditions.

We welcome applications from a variety of groups, and are interested in a range of proposals.

In response to the question:

Are you expecting respondents to have a clinical background?

We are able to work with Self Leadership tools and processes as well as being able to provide ongoing coaching and support but without the background to be providing any sort of clinical advice. I see that this might be a useful situation in that those people with critical conditions who needed clinical advice would always be referred back to their treatment providers (with our support and collaboration) for such advice and information. Is this how you see it or are you looking for specific medical expertise?

No, we do not expect respondents to have a clinical background. Working with self management tools, providing coaching and support etc is an important approach/component of self-management. We would be pleased to receive expressions of interest that cover these components.

In response to the question:

Is there any more information anywhere re scope, budget etc. Not clear about scale (is it infrastructure or implementation that you are looking for?).

The budget allocated is $80,000 sustainable funding. I envisage that this will be used for some ’generic’ self management programmes and will also be used to support a variety of smaller one off projects with Non Government Organisations (NGO)s and self help groups for example. (It really depends what comes in from the REOI)

The funding supports self management training across a whole range of different conditions while also recognising and supporting ‘niche’ or tailored programmes for different conditions for example neurological, respiratory and different sub-populations for example Maori, Pacific, refugee, elderly, children.

The C&C DHB support the skills of self-management that enables people to adapt treatment to their condition by improving their knowledge, skills and confidence to take increasing control of their own condition and integrate effective self-management into their daily lives.

Successful chronic care management programmes focus on providing extra resources and activities that enhance patient self management and monitoring, empowering the patient to take a lead role in managing their health and ensuring access to the range of services and resources required to achieve optimal outcomes. It is across these areas that C&C DHB aspires to be more systematic and effective.

We are quite flexible and have not put any limits on people numbers.

The programme could be delivered to a variety of groups in the sector: newly diagnosed with a long-term condition, frequent users of hospital and people across the whole continuum.
