Capital and Coast DHB has recently completed a report on the gains made in primary health care since the beginning of PHOs in 2004 when we developed a framework to measure the early impact and effectiveness of our investment in primary care. We have drawn on a wide range of data and expertise to develop the report, which has been peer reviewed by public health specialists at Auckland and Otago Universities and Counties Manukau DHB.This report provides robust evidence of the contribution that primary health care makes to lower avoidable hospitalisation, reduced ED use and improved health outcomes. It also demonstrates how primary care can reduce disparities in health outcomes through access to quality health services and action to address the determinants of health in and with the communities experiencing barriers to good health.
