Capital PHO
Capital PHO provides primary health care services to people living throughout Wellington’s suburbs from Seatoun through to Johnsonville and Newlands. Capital PHO is a not-for-profit charitable trust.
Approximately 155,000 people, or 93% of Wellington’s population are enrolled with one of Capital PHO’s 32 member practices. This includes over 9,000 Maori and over 5,000 Pacific peoples. The role of Capital PHO’s board is to oversee the setting of policy, provide direction to management and monitor progress and performance. The Board meet monthly and meetings are open to the public. Capital PHO has also implemented the following services to respond to the needs of its community:
- Employed outreach nurses and support workers to work with patients in the community who need additional support to access primary care
- Provides free interpreter services for primary care visits
- Provide free GP visits for high need and low income people who need urgent GP-level care
- Provided cultural awareness training to primary care providers throughout our community
- Provided a range of health promotion activities that are based on a “healthy lifestyle” theme
- The PHO team work closely with a range of NGO providers, sharing planning and implementation of services responding to health and social service needs
- Implemented Care Plus and Enhanced Care Plus for patients with complex health needs.
