Who are we?

Capital Coast Rehab is a specialised Assessment, Treatment and Rehabilitation (AT&R) service covering Wellington and the Kapiti Coast . We provide both treatment both in the Community and Inpatient as well as offeriong a Pyschogeriatric service.

What do we do?

We work in partnership with people to maximise abilities and optimise lifestyles following illness surgery or injury from an accident.

We provide individual Assessment, Treatment and Rehabilitation programmes for people aged 16 years and over who live in our region and have disabilities arising from...

• Stroke
• Complex musculoskeletal and orthopaedic injury and illness including amputations
• Complex age related illness and disease that would benefit from multidisciplinary Assessment, Treatment and Rehabilitation
• Neurological illness
• Traumatic brain injury
• Psychogeriatric conditions (65 years and older)
• ...and who have been assessed by members of the team as having reasonable potential for functional improvement within a specified time frame.

What is rehabilitation?

Rehabilitation is restoring and relearning lost abilities or finding new ways of doing everyday activities to maximise ones lifestyle.

Successful rehabilitation is a partnership between the client and health professionals involved, and is very different from being ‘treated’ in hospital or at home. In all areas of our service, highly trained nursing, therapy, medical and technical staff work closely with clients, families/whanau and friends, to design and implement individualised rehabilitation programmes.

All clients are encouraged to do as much for themselves as they can safely do.

Who can refer to Capital Coast Rehab?

Referrals are accepted from a range of sources, including:

• General Practitioners and other primary health care providers
• Inpatient and outpatient services at Capital Coast Health other District Health Boards
• Any health professional
• Capital Support (Needs Assessment and Service Coordination Agencies)
• Community Support Organisations
• Family/whanau and self referrals

We encourage referrals from individuals, family members and community support organisation’s following consultation with or in conjunction with the individual’s General Practitioner. To send a referral, please use the contact details available in the Contact Us section.

What about family and friends?

Capital Coast Rehab recognises the important role that family/whanau and friends play in the rehabilitation process.

Family and friends will be invited to participate in therapy sessions as appropriate and agreed with the client. As required, family meetings will occur to encourage participation in the rehabilitation process and ensure good communication between the client, team members and family/whanau.
