What is rehabilitation?
Rehabilitation is restoring and relearning lost abilities or finding new ways of doing everyday activities to maximise ones lifestyle.
Successful rehabilitation is a partnership between the client and health professionals involved, and is very different from being ‘treated’ in hospital or at home. In all areas of our service, highly trained nursing, therapy, medical and technical staff work closely with clients, families/whanau and friends, to design and implement individualised rehabilitation programmes.
All clients are encouraged to do as much for themselves as they can safely do.
Who can refer to Capital Coast Rehab?
We encourage referrals from individuals, family members and community support organisation’s following consultation with or in conjunction with the individual’s General Practitioner. For people over the age of 65, referrals should be made via the care coordination centre otherwise, to send a referral, please use the contact details available in the Contact Us section.
What about family and friends?
Capital Coast Rehab recognises the important role that family/whanau and friends play in the rehabilitation process.
Family and friends will be invited to participate in therapy sessions as appropriate and agreed with the client. As required, family meetings will occur to encourage participation in the rehabilitation process and ensure good communication between the client, team members and family/whanau
