Community Team

What is the community team?

It is an inter-professional health team who work together to ensure that assessment, treatment and rehabilitation needs and goals for our clients are met. The team is based at three sites – Wellington and Kenepuru Hospitals and Kapiti health centre. The aim of each community team is to support the client, as part of their family/whanau to achieve their full potential within their own community.

The community team is hospital-based, but has strong links with community groups in order to provide an integrated service.

Who is in the team?

  • Liaison nurses
  • Occupational therapists
  • Physiotherapists
  • Rehab assistants and workshop technician
  • Social workers
  • Speech-language therapists
  • Family therapist
  • Neuropsychologist
  • Medical specialists (ie. Geriatricians and a Rehabilitation Medicine Specialist)
  • Access to other health professionals, eg. dietitian

What services do the community team provide?

We provide assessment to determine appropriate treatment/rehabilitation for each client.

Depending on the individual’s needs, the community team offers a specialised rehabilitation programme. This may include:

  • Community or home visits
  • Outpatient treatments
  • Worksite assessments and return to work planning
  • Co-ordination of the rehabilitation programme
  • Support and advocacy for the client and their family/whanau/carers
  • Facilitated access to community services, groups and organisations
  • Specialist ACC contracted rehabilitation (eg. Training for independence programs)
