Community Health Service
Community Health Service (CHS), in partnership with the Therapies Service, provides inter-disciplinary professional services and home support services to patients with a personal health need living in their own community.
As hospitals work towards a model of ambulatory care and early discharge, CHS increasingly provides the acute nursing services once provided in hospital.
CHS has a team of skilled nurses who work along side other community providers to ensure the needs of the individual are met and patients receive specialist nursing services in their own homes or at clinics located at Wellington Hospital, Kenepuru Community Hospital and Kapiti Health Centre sites.
The clinics are for community health patients who require specialist nursing services that can not be met by their doctor, but who are able to access a clinic.
The service also has a team of Health Care assistants at Wellington Hospital , Kenepuru Community hospital and Kapiti Health Centre who work closely with District Nurses to provide home help / personal care.
