Health of Older People & Palliative Care - To manage contracts for a wide range of services. To plan and fund home and community services for older people and people with chronic illness.
Maori Health Development Group
Mental Health - To manage contracts and relationships with community based mental health and addiction treatment service providers. The mental health team also have responsibility to coordinate the implementation of the Journey Forward, the five year service development plan for mental health and addictions.
Organisational Planning - To provide leadership to the planning team and provide support to the Executive Management Team so that the team is motivated and committed to a common direction.
Pacific Health - To provide advice to the DHB Board, Executive Management Team and the wider organisation in respect to driving organisational responsiveness to Pacific health issues.
Population Health & Intersectoral Strategy - To assist the DHB to maximise its overall population health gain across its key priority areas.
Primary, Community Care & Long Term Conditions (LTC) Management - To prevent disease, create & monitor screening programmes, support health promotion and action to change the social and physical environments to make healthy choices easier.
Primary Health Care Nursing - To improve the patient journey between C&C DHB's primary, secondary and tertiary services. There are an estimated 500 primary health care nurses in C&C DHB. They work in a variety of services including: general practice, Public Health Services, schools, prison services, reproductive and sexual health, palliative care, Aged Care facilities, NGO support services, youth health, Maori or Pacific provider organisations and well-child services to name a few.
Secondary / Tertiary, Operations and Analysis - Responsible for the portfolio management of secondary / tertiary hospital services, oral health, community pharmacy services, and the community laboratory services.
