Long Term Conditions (LTC) Management

The attached documents ‘Long Term Conditions (LTC) Management Framework and Action Plans’ have been developed with Primary Health Organisations (PHOs) and key stakeholders from the Hospital Health Service (HHS), to circulate widely throughout the District. They are mainly focussed on joint work around existing programmes for process improvement, to improve outcomes and reduce disparities, informing the next three years, and also Capital & Coast District Health Board (C&CDHB) District Annual plans for 2008-2011.

Background

The development of a LTC Management Framework and Action Plan originated from the Executive Management Team to assist with a shared understanding of the scope, the approach, what we hope to achieve or influence and practical areas for joint work. The anticipated value of the LTC Management Framework and Action Plan is in raising the profile of chronic conditions within a busy sector, developing consensus about areas for collective action.

The LTC Management Framework and Action Plan will assist in delivering on C&CDHB’s two high level outcomes (Health Goals):

  • Reducing disparities in health status
  • Reducing the incidence and impact of chronic conditions

The two outcomes are interlinked, given the disproportionate prevalence and impact of chronic conditions on Maori, Pacific and low income populations and other sub-populations with poorer health status.

The LTC Management Framework and Action Plans recognise important related strategic and operational plans that contribute to progress in this area and recognise the substantial investment, range of services and initiatives already in place and part of ‘every day business in the health sector that contribute to chronic care management and prevention of chronic conditions.

The Framework and Action Plans contribute to a shared vision, an explicit set of actions and a general guide for resource allocation over the next 3 years.

The Plan is not exclusive but identifies areas where we, as a DHB, with other agencies, providers, consumer groups, families and communities aim, through collective effort, to make systematic improvements in services, outcomes and to reduce disparities.
